Your first visit to Matrix Physical Therapy will be your initial evaluation.   At this time, we will assess the current limitations, history and exacerbating features of your injury, and devise an individual plan to address these.

This visit will last approximately sixty minutes.

• Please bring comfortable clothing with you.   If you are being seen for a knee or a hip problem, please bring shorts to allow the area to be properly visualized.   If you are to be seen for your shoulder, please bring a t-shirt or tank top.   It is not necessary to bring sneakers to this visit.

• Your insurance card, a photo ID, and the prescription from your physician are required. The state laws of New Jersey do not allow us to treat you without a valid prescription. The prescription expires after the number of weeks specified, from the date written. If no time frame is specified, then the prescription expires 30 days from the date written.

• The forms you will need are available to download below. Please fill them out prior to your appointment time. If you are unable to download them, or fill them out, arrive at your appointment approximately 15 minutes before your appointment time.
